Routine vaccines

Recommendations.

Make sure you are up-to-date on all routine vaccines before every trip. Some of these vaccines include

  • Chickenpox (Varicella)
  • Diphtheria-Tetanus-Pertussis
  • Flu (influenza)
  • Measles-Mumps-Rubella (MMR)

Hepatitis A

Recommended for unvaccinated travelers one year old or older going to Montenegro.

Infants 6 to 11 months old should also be vaccinated against Hepatitis A. The dose does not count toward the routine 2-dose series.

Travelers allergic to a vaccine component should receive a single dose of immune globulin, which provides effective protection for up to 2 months depending on dosage given.

Unvaccinated travelers who are over 40 years old, immunocompromised, or have chronic medical conditions planning to depart to a risk area in less than 2 weeks should get the initial dose of vaccine and at the same appointment receive immune globulin.

Recommended for unvaccinated travelers of all ages traveling to Montenegro.

Cases of measles are on the rise worldwide. Travelers are at risk of measles if they have not been fully vaccinated at least two weeks prior to departure, or have not had measles in the past, and travel internationally to areas where measles is spreading.

All international travelers should be fully vaccinated against measles with the measles-mumps-rubella (MMR) vaccine, including an early dose for infants 6–11 months, according to  CDC’s measles vaccination recommendations for international travel .

Dogs infected with rabies are not commonly found in Montenegro.

Rabies is present in some terrestrial wildlife species.

If rabies exposures occur while in Montenegro, rabies vaccines are typically available throughout most of the country.

Rabies pre-exposure vaccination considerations include whether travelers 1) will be performing occupational or recreational activities that increase risk for exposure to potentially rabid animals and 2) might have difficulty getting prompt access to safe post-exposure prophylaxis.

Please consult with a healthcare provider to determine whether you should receive pre-exposure vaccination before travel.

Avoid contaminated water

How most people get sick (most common modes of transmission)

  • Touching urine or other body fluids from an animal infected with leptospirosis
  • Swimming or wading in urine-contaminated fresh water, or contact with urine-contaminated mud
  • Drinking water or eating food contaminated with animal urine
  • Avoid contaminated water and soil
  • Avoid floodwater

  • Avoid rodents and areas where they live
  • Avoid sick people

Tuberculosis (TB)

  • Breathe in TB bacteria that is in the air from an infected and contagious person coughing, speaking, or singing.

Eat and drink safely

Food and water standards around the world vary based on the destination. Standards may also differ within a country and risk may change depending on activity type (e.g., hiking versus business trip). Prevent bug bites

Bugs (like mosquitoes, ticks, and fleas) can spread a number of diseases in Montenegro. Many of these diseases cannot be prevented with a vaccine or medicine. You can reduce your risk by taking steps to prevent bug bites.

What can I do to prevent bug bites?

  • Cover exposed skin by wearing long-sleeved shirts, long pants, and hats.
  • Use an appropriate insect repellent (see below).
  • Use permethrin-treated clothing and gear (such as boots, pants, socks, and tents). Do not use permethrin directly on skin.
  • Stay and sleep in air-conditioned or screened rooms.
  • Use a bed net if the area where you are sleeping is exposed to the outdoors.

What type of insect repellent should I use?

  • FOR PROTECTION AGAINST TICKS AND MOSQUITOES: Use a repellent that contains 20% or more DEET for protection that lasts up to several hours.
  • Picaridin (also known as KBR 3023, Bayrepel, and icaridin)
  • Oil of lemon eucalyptus (OLE) or para-menthane-diol (PMD)
  • 2-undecanone
  • Always use insect repellent as directed.

What should I do if I am bitten by bugs?

  • Avoid scratching bug bites, and apply hydrocortisone cream or calamine lotion to reduce the itching.
  • Check your entire body for ticks after outdoor activity. Be sure to remove ticks properly.

What can I do to avoid bed bugs?

Although bed bugs do not carry disease, they are an annoyance. See our information page about avoiding bug bites for some easy tips to avoid them. For more information on bed bugs, see Bed Bugs .

Stay safe outdoors

If your travel plans in Montenegro include outdoor activities, take these steps to stay safe and healthy during your trip.

  • Stay alert to changing weather conditions and adjust your plans if conditions become unsafe.
  • Prepare for activities by wearing the right clothes and packing protective items, such as bug spray, sunscreen, and a basic first aid kit.
  • Consider learning basic first aid and CPR before travel. Bring a travel health kit with items appropriate for your activities.
  • If you are outside for many hours in heat, eat salty snacks and drink water to stay hydrated and replace salt lost through sweating.
  • Protect yourself from UV radiation : use sunscreen with an SPF of at least 15, wear protective clothing, and seek shade during the hottest time of day (10 a.m.–4 p.m.).
  • Be especially careful during summer months and at high elevation. Because sunlight reflects off snow, sand, and water, sun exposure may be increased during activities like skiing, swimming, and sailing.
  • Very cold temperatures can be dangerous. Dress in layers and cover heads, hands, and feet properly if you are visiting a cold location.

Stay safe around water

  • Swim only in designated swimming areas. Obey lifeguards and warning flags on beaches.
  • Practice safe boating—follow all boating safety laws, do not drink alcohol if driving a boat, and always wear a life jacket.
  • Do not dive into shallow water.
  • Do not swim in freshwater in developing areas or where sanitation is poor.
  • Avoid swallowing water when swimming. Untreated water can carry germs that make you sick.
  • To prevent infections, wear shoes on beaches where there may be animal waste.

Keep away from animals

Most animals avoid people, but they may attack if they feel threatened, are protecting their young or territory, or if they are injured or ill. Animal bites and scratches can lead to serious diseases such as rabies.

Follow these tips to protect yourself:

  • Do not touch or feed any animals you do not know.
  • Do not allow animals to lick open wounds, and do not get animal saliva in your eyes or mouth.
  • Avoid rodents and their urine and feces.
  • Traveling pets should be supervised closely and not allowed to come in contact with local animals.
  • If you wake in a room with a bat, seek medical care immediately. Bat bites may be hard to see.

All animals can pose a threat, but be extra careful around dogs, bats, monkeys, sea animals such as jellyfish, and snakes. If you are bitten or scratched by an animal, immediately:

  • Wash the wound with soap and clean water.
  • Go to a doctor right away.
  • Tell your doctor about your injury when you get back to the United States.

Consider buying medical evacuation insurance. Rabies is a deadly disease that must be treated quickly, and treatment may not be available in some countries.

Reduce your exposure to germs

Follow these tips to avoid getting sick or spreading illness to others while traveling:

  • Wash your hands often, especially before eating.
  • If soap and water aren’t available, clean hands with hand sanitizer (containing at least 60% alcohol).
  • Don’t touch your eyes, nose, or mouth. If you need to touch your face, make sure your hands are clean.
  • Cover your mouth and nose with a tissue or your sleeve (not your hands) when coughing or sneezing.
  • Try to avoid contact with people who are sick.
  • If you are sick, stay home or in your hotel room, unless you need medical care.

Avoid sharing body fluids

Diseases can be spread through body fluids, such as saliva, blood, vomit, and semen.

Protect yourself:

  • Use latex condoms correctly.
  • Do not inject drugs.
  • Limit alcohol consumption. People take more risks when intoxicated.
  • Do not share needles or any devices that can break the skin. That includes needles for tattoos, piercings, and acupuncture.
  • If you receive medical or dental care, make sure the equipment is disinfected or sanitized.

Know how to get medical care while traveling

Plan for how you will get health care during your trip, should the need arise:

  • Carry a list of local doctors and hospitals at your destination.
  • Review your health insurance plan to determine what medical services it would cover during your trip. Consider purchasing travel health and medical evacuation insurance.
  • Carry a card that identifies, in the local language, your blood type, chronic conditions or serious allergies, and the generic names of any medications you take.
  • Some prescription drugs may be illegal in other countries. Call Montenegro’s embassy to verify that all of your prescription(s) are legal to bring with you.
  • Bring all the medicines (including over-the-counter medicines) you think you might need during your trip, including extra in case of travel delays. Ask your doctor to help you get prescriptions filled early if you need to.

Many foreign hospitals and clinics are accredited by the Joint Commission International. A list of accredited facilities is available at their website ( www.jointcommissioninternational.org ).

In some countries, medicine (prescription and over-the-counter) may be substandard or counterfeit. Bring the medicines you will need from the United States to avoid having to buy them at your destination.

Select safe transportation

Motor vehicle crashes are the #1 killer of healthy US citizens in foreign countries.

In many places cars, buses, large trucks, rickshaws, bikes, people on foot, and even animals share the same lanes of traffic, increasing the risk for crashes.

Be smart when you are traveling on foot.

  • Use sidewalks and marked crosswalks.
  • Pay attention to the traffic around you, especially in crowded areas.
  • Remember, people on foot do not always have the right of way in other countries.

Riding/Driving

Choose a safe vehicle.

  • Choose official taxis or public transportation, such as trains and buses.
  • Ride only in cars that have seatbelts.
  • Avoid overcrowded, overloaded, top-heavy buses and minivans.
  • Avoid riding on motorcycles or motorbikes, especially motorbike taxis. (Many crashes are caused by inexperienced motorbike drivers.)
  • Choose newer vehicles—they may have more safety features, such as airbags, and be more reliable.
  • Choose larger vehicles, which may provide more protection in crashes.

Think about the driver.

  • Do not drive after drinking alcohol or ride with someone who has been drinking.
  • Consider hiring a licensed, trained driver familiar with the area.
  • Arrange payment before departing.

Follow basic safety tips.

  • Wear a seatbelt at all times.
  • Sit in the back seat of cars and taxis.
  • When on motorbikes or bicycles, always wear a helmet. (Bring a helmet from home, if needed.)
  • Avoid driving at night; street lighting in certain parts of Montenegro may be poor.
  • Do not use a cell phone or text while driving (illegal in many countries).
  • Travel during daylight hours only, especially in rural areas.
  • If you choose to drive a vehicle in Montenegro, learn the local traffic laws and have the proper paperwork.
  • Get any driving permits and insurance you may need. Get an International Driving Permit (IDP). Carry the IDP and a US-issued driver's license at all times.
  • Check with your auto insurance policy's international coverage, and get more coverage if needed. Make sure you have liability insurance.
  • Avoid using local, unscheduled aircraft.
  • If possible, fly on larger planes (more than 30 seats); larger airplanes are more likely to have regular safety inspections.
  • Try to schedule flights during daylight hours and in good weather.

Medical Evacuation Insurance

If you are seriously injured, emergency care may not be available or may not meet US standards. Trauma care centers are uncommon outside urban areas. Having medical evacuation insurance can be helpful for these reasons.

Maintain personal security

Use the same common sense traveling overseas that you would at home, and always stay alert and aware of your surroundings.

Before you leave

  • Research your destination(s), including local laws, customs, and culture.
  • Monitor travel advisories and alerts and read travel tips from the US Department of State.
  • Enroll in the Smart Traveler Enrollment Program (STEP) .
  • Leave a copy of your itinerary, contact information, credit cards, and passport with someone at home.
  • Pack as light as possible, and leave at home any item you could not replace.

While at your destination(s)

  • Carry contact information for the nearest US embassy or consulate .
  • Carry a photocopy of your passport and entry stamp; leave the actual passport securely in your hotel.
  • Follow all local laws and social customs.
  • Do not wear expensive clothing or jewelry.
  • Always keep hotel doors locked, and store valuables in secure areas.
  • If possible, choose hotel rooms between the 2nd and 6th floors.

It’s best to be prepared to prevent and treat common illnesses and injuries. Some supplies and medicines may be difficult to find at your destination, may have different names, or may have different ingredients than what you normally use.

Healthcare in Montenegro

FCDO  has a list of  medical providers in Montenegro where some staff will speak English.

Health insurance cards

Montenegro recognises the Global Health Insurance Card ( GHIC ).

You will need to take your British passport and GHIC to a local office of the Montenegrin Health Insurance Fund (in Montenegrin) to request a certificate of entitlement to medical care. You need this in advance of medical treatment, or as soon as possible after an urgent admission to hospital. If you do not have a certificate, the hospital will charge for the treatment and paid bills will not be reimbursed.

Any treatment provided is on the same terms as Montenegrin nationals. You’ll have to pay for your prescribed medicines and for other medical treatment.

A GHIC is not an alternative to travel insurance. You may have costs your GHIC does not cover.

Cross border Montenegro

Crossing the border outside the border crossings

If you are planning a hiking tour that includes crossing the border outside the border crossings (for example: on Prokletije, Hajla, Ljubišnja, Kamena Gora, Maglić, Orjen…), you need to follow the legal procedure.

You will need to send the request for crossing the border crossing to the relevant institution, ie the border police of the country you are leaving. The border police of the country of exit will take all necessary measures to ensure your smooth crossing.

Transporting pets

In accordance with the Ordinance on the conditions for non-commercial transportation of pets, pets such as dogs and cats can be transported non-commercially (up to 5 pets, not younger than 3 months) if:

  • they are identified in the prescribed manner
  • they are accompanied by a passport
  • they have been vaccinated against rabies
  • they are older than 12 weeks on the day of vaccination
  • 21 days have elapsed since the day of the first rabies vaccination 
  • the rabies antibody titration test was performed 30 days after the date of vaccination and at least three months before the date of non-commercial movement (for non-EU countries)
  • they were treated against Echinococcus multilocularis in the prescribed manner upon entry or exit from Montenegro 
  • they are accompanied by a veterinary health certificate.

The entry of pets is approved if they are accompanied by a veterinary passport and the appropriate veterinary certificate for non-commercial movement of pets, issued by a licensed veterinarian of the exporting country.

Registration of residence

During your stay in Montenegro, you are obliged to register and deregister your stay.

If you are staying in a hotel, private accommodation, or any other facility, the host is obliged to register your stay within 12 hours of your arrival and check out of your stay.

If you are a foreigner and you are staying in Montenegro for tourism purposes, and you do not use the registration services of the accommodation provider, you can report your stay to the local tourist organization or police in the place where you are staying, or through the competent local government within 24 hours.

If you are staying in several municipalities in Montenegro, you are obliged to register your stay in each municipality where you stay for more than 24 hours. Any application to stay is subject to control and examination.

TREATMENT PROCESS

The treatments are made in JCI certified, high-tech hospitals where the best doctors work. On the day of treatment, the airport or hotel transfer is made by the team.

Montenegro as a high-quality health tourism destination: Trends and Perspectives

The main objective of the paper is to provide a theoretical and practical framework in order to identify the key advantages and disadvantages of Montenegro as a health tourism destination. The main method that was used was a SWOT analysis, as well as a survey based on an original and thus self designed questionnaire intended for this purpose. The results of the research have shown some key advantages and disadvantages of Montenegrin health-tourism. Although Montenegro as a country, has a great potential regarding the health tourism sector, it is a fact that this potential is not adequately valorized. In that sense, a strategy of development of the health tourism sector should be created. Strategy should contain a specific vision, mission, strategic goals, as well as measures that should be carried out and activities that should be realized in order to make Montenegro attractive and high-quality health tourism destination.

The hasty practice of moving to foreign countries for high tech medical services is hinged on a diverse range of motivators of which developing countries like Nigeria can take advantage of. This paper explored the reasons behind Nigerian consumers' outbound medical tourism; identified the risk factors that give Nigerians concern over engaging in medical tourism; and determined the effect of cost of treatment on outbound medical tourism. Using a structured online questionnaire, data were obtained from a judgemental sample of 100 medical tourists. The data gathered were analysed using descriptive statistics and linear regression analysis. The findings showed that Nigerians undertake outbound medical tourism due to improper medical treatment in home country, infrastructural inadequacies, commensurate cost of treatment in destination countries and availability of qualified health service providers abroad. Tourism destination competitiveness (TDC) has become an important research topic and has received considerable attention from many researchers. However, according to the authors, no study has yet been conducted to measure the perception of the supply-side stakeholders concerning the TDC of Bangladesh. The purpose of this study is to measure the perceived destination competitiveness by identifying the attributes appropriate to Bangladesh. The defined attributes were empirically inspected by importance-performance analysis (IPA) tool through primary data from 303 practitioners. A stratified sampling method was used in this regard. The IPA findings reveal there are only a few attributes like physiography and climate, hospitality, geographic location, and cuisine in which Bangladesh tourism is performing well against its major perceived competitive destinations like India, Malaysia, Nepal, and Thailand. However, having important destination attributes mainly from nature, destination Bangladesh is still behind its perceived competitors. This research creates the value of accepting a destination's competitiveness attributes that advances destination theory. The findings offer significant implications for Bangladeshi policymakers for developing a strategic marketing plan and identifying priorities for resource allocation in tourism. The study concludes with mentioning few limitations while highlighting areas for further research aimed at developing a tourism destination competitiveness (TDC) model for destination Bangladesh.

The study builds a case for future growth of transformative travel in South Africa driven by Cultural Creatives, the new silent class of conscious consumers in a quest for personal transformation through authentic and immersive tourist experiences. The main objective of the study was to identify preferred activities and experiences of Cultural Creatives in Soweto which is expected to drive the future growth of transformative tourism in South Africa. This qualitative study was based on 20 semi structured interviews with international tourists visiting Soweto in the period 3 January to 30 March 2018. Interviews were transcribed verbatim and thematic content analysis was carried out in identifying tourist activities and experiences most closely aligned to emerging themes of transformative tourism as: individual experiential walking through the township; living as the locals do; and dining with the locals. The results validate the need for clear alignment of South African destinations' offerings with Cultural Creatives' transmodern values and worldviews in facilitating the future growth of transformative tourism in South Africa.

In Italy, in 2017, about 33,000 hotels operated, which characterize an evolving offer that tends to favour hotels with better services than in the past, also in response to the growing competition from less expensive non-hotel businesses. This paper is dedicated to the study of income dynamics of larger Italian hotels with a turnover of over € 800,000, analyzing the tenyear trend of five balance sheet indices of a significant sample of companies. United States Government’s Response to the Clade I Mpox Outbreak in the Democratic Republic of the Congo and Other Countries in the Region

This week, the Africa Centres for Disease Control and Prevention (Africa CDC) declared the mpox clade I outbreak a Public Health Emergency of Continental Security, and the World Health Organization (WHO) declared this outbreak a Public Health Emergency of International Concern. The United States government supports those declarations. The United States will continue to work closely with African governments, Africa CDC and WHO to ensure an effective response to the current outbreak and to protect the health and lives of people of the region.

In 2022, the world experienced a global outbreak of clade II mpox, which led to more than 95,000 cases across 115 non-endemic countries. Clade I mpox tends to cause a higher number of severe infections and have a higher mortality rate than clade II mpox. The evidence for clade I mpox clinical outcomes is based primarily on data from endemic countries, particularly DRC. We expect it would cause lower morbidity and mortality in the United States than in the DRC.

DRC is currently experiencing the largest number of annual suspect cases ever recorded and the disease has now been identified in several neighboring countries where mpox (clade I or clade II) has not been found in the past.

U.S. Government Partnership on the Mpox Response

The United States Government has been closely monitoring the spread of clade I mpox in the DRC and neighboring countries since 2023, and we have been working closely with the Government of DRC, as well as regional and global health partners to reduce the impact of this outbreak and safeguard public health. U.S. government support for the mpox response also builds on our robust, longstanding health partnerships with DRC and throughout Africa, which have helped combat infectious diseases such as HIV, tuberculosis, and malaria for over 20 years. In fiscal year 2023, the United States allocated more than $2.65 billion in bilateral health funding in Central and Eastern Africa. This response also builds on a longstanding partnership on global health security between DRC and the United States.

In addition to ongoing health support, in the last few months the United States has provided an additional $17 million USD to support clade I mpox preparedness and response efforts in Central and Eastern Africa. The funding has enabled stronger surveillance, risk communication, and community engagement, as well as needed laboratory supplies and diagnostics, clinical services, and vaccine planning.

Vaccination will be a critical element of the response to this outbreak. To support this effort, the United States is donating 50,000 doses of the Food and Drug Administration (FDA)-approved JYNNEOS vaccine to DRC. The United States is working with other countries, WHO, and international partners to encourage donations that support vaccine efforts and address challenges to vaccine delivery.

Mpox Preparedness for the United States

The risk to the general public in the United States from clade I mpox circulating in the DRC is very low, and there are no known cases in the United States at this time. Due to efforts over the last nine months, the United States is well prepared to rapidly detect, contain, and manage clade I cases should they be identified domestically. The United States has a robust surveillance system in place, including through clinical testing and wastewater analysis. We continue to encourage those at high risk to get vaccinated with the JYNNEOS mpox vaccine, which has been demonstrated to be safe and highly effective at preventing severe disease from mpox. Those who have already had clade II mpox or are fully vaccinated against mpox are expected to be protected against severe illness from clade I mpox.

CDC has issued an updated Health Alert Network advisory urging clinicians to consider clade I mpox in people who have been in DRC or neighboring countries in the previous 21 days; clinicians are also asked to submit specimens for clade-specific testing for these patients if they have symptoms consistent with mpox. Given the geographic spread of clade I mpox, the U.S. CDC issued an updated Travel Health Notice on Aug. 7, 2024, recommending travelers to DRC and neighboring countries practice enhanced precautions.

Mpox outbreaks in Africa are a global health emergency, WHO declares

The mpox outbreaks in Africa are a global emergency , with cases confirmed among children and adults in more than a dozen countries, the World Health Organization declared Wednesday.

The declaration comes after the Africa Centers for Disease Control and Prevention announced earlier this week that the outbreaks of mpox ( formerly known as monkeypox ) were a public health emergency , with more than 500 deaths, and called for international help to stop the virus' spread.

"This is something that should concern us all. ... The potential for further spread within Africa and beyond is very worrying," said WHO director-general Tedros Adhanom Ghebreyesus. A global emergency is WHO's highest level of alert but the designation does not necessarily mean a disease is particularly transmissible or lethal.

With a new form of the virus spreading and few vaccine doses available on the continent, the Africa CDC warned the virus might spill across international borders.

"We are now in a situation where (mpox) poses a risk to many more neighbors in and around central Africa," said Salim Abdool Karim, a South African infectious diseases expert who chairs the Africa CDC emergency group. He said the new version of mpox spreading from Congo appears to have a death rate of about 3-4%.

Mpox has been detected in 13 countries this year with more than 96% of all cases and deaths are in Congo, the Africa CDC previously said.

Cases are up 160% and deaths are up 19% compared with the same period last year. So far, there have been more than 14,000 cases and 524 people have died.

Officials at the Africa CDC said nearly 70% of cases in Congo are in children younger than 15, who also accounted for 85% of deaths.

Where has the mpox outbreak spread?

Sweden reported its first case of the more infectious form of the virus on Thursday, just a day after the WHO declaration.

The patient recently sought health care in Stockholm, the Swedish public health agency said  in a statement .

"In this case a person has been infected during a stay in the part of Africa where there is a major outbreak of (the more infectious mpox)," the agency said.

The person had been treated and given "rules of conduct," said Magnus Gisslen, a state epidemiologist with the Swedish health agency.

"The fact that a patient with mpox is treated in the country does not affect the risk to the general population," officials said, adding that experts estimate that risk to be "very low," though occasional imported cases may continue to occur.

What is mpox?

Mpox, which was previously known as monkeypox , is in the same family of viruses that includes variola virus, which causes smallpox; vaccinia virus, used in the smallpox vaccine; and cowpox virus,  according to  the Centers for Disease Control and Prevention. It was discovered in a colony of monkeys in 1958 and its first human case was recorded in 1970 in the Democratic Republic of Congo, but has been reported in humans in other countries.

In 2022, the WHO  declared mpox a global emergency after it spread to more than 70 countries, mostly affecting gay and bisexual men. In that outbreak, fewer than 1% of people died.

The current outbreak is a different strain than in 2022, Dr. Céline Gounder, a CBS News medical contributor and editor-at-large for public health at KFF Health News, explained on CBS "Mornings" Thursday .

"The strain is more mortal, so in other words, causes more percentage of deaths. It's also more transmissible. It spreads more easily from person to person," she said. 

New form of mpox symptoms

Earlier this year, scientists reported the emergence of a new form of mpox, which can kill up to 10% of people infected, in a Congolese mining town that they feared might spread more easily.  Mpox mostly spreads via close contact with infected people, including through sex. 

Unlike in previous mpox outbreaks , where lesions were mostly seen on the chest, hands and feet, the new form causes milder symptoms and lesions on the genitals. That makes it harder to spot, meaning people might also sicken others without knowing they're infected.

"That last outbreak that we saw about two years ago was primarily being spread among men who have sex with men, bisexual men, transgender people. What we're seeing this time is some 85% of the cases are among kids, and so it's the poor hygiene that you see among little kids playing with each other, skin-to-skin contact — that's how we're seeing this being transmitted now," Gounder said. 

She added that the current risk for infection in the United States is "very low."

"We have yet to see any cases here, so I don't think you should get too alarmed. If you see a child who looks like they have the chickenpox, it probably is the chickenpox. But that's what the mpox rash looks like," Gounder said. "The disease is more severe in that you have worse flu-like symptoms. You might also have swollen lymph nodes, but the rash is quite similar to the chickenpox."

Can we stop the spread of mpox?

Western countries during the 2022 outbreak mostly shut down the spread of mpox with the help of vaccines and treatments, but very few of those have been available in Africa.

Michael Marks, a professor of medicine at the London School of Hygiene and Tropical Medicine, said that in the absence of mpox vaccines licensed in the West, officials could consider inoculating people against smallpox, a related disease. 

"We need a large supply of vaccine so that we can vaccinate populations most at risk," he said, adding that would mean sex workers, children and adults living in outbreak regions.

Gounder said watching for symptoms, testing, and doing surveillance to spot the spread early can help curb the disease. 

"A lot of what we've been doing as the United States has been supporting the Democratic Republic of Congo, which is where the majority of these cases we've been supporting them to do that surveillance, to do that diagnostic testing, so you can isolate and prevent further spread, further transmission," she said.

WHO declares 2024 mpox surge a 'public health emergency of international concern'

The World Health Organization has declared mpox a public health emergency of international concern. Cases of mpox — previously called monkeypox — have been surging in the Democratic Republic of Congo. In recent weeks, cases have appeared in nearby African countries, including several that have never reported mpox cases before.

"What we're seeing is tip of the iceberg" because of weaknesses in the surveillance system, says Dr. Dimie Ogoina , the chair of the emergency committee convened by WHO and an infectious disease physician at Niger Delta University in Nigeria.

"This is something that should concern us all," says WHO's director-general, Tedros Adhanom Ghebreyesus.

WHO has declared seven public health emergencies in the past, including one for mpox in 2022. The type of mpox that is circulating now is known to be more deadly than the type that swept the globe two years ago.

"We need to be ... aggressive"

On Tuesday, the Africa Centres for Disease Control and Prevention took a similar step, declaring mpox an emergency.

Africa CDC has never done anything like this before.

"We can no longer be reactive — we need to be proactive and aggressive,” says Dr. Jean Kaseya , director-general of Africa CDC. "This is a fight for all Africans and we will fight it together."

In the Democratic Republic of Congo (DRC), children make up the majority of the 14,000 reported cases and 511 deaths so far in 2024. Those numbers roughly match the number of cases reported in all of last year in the country — and they dwarf the mpox numbers reported in 2022.

"[The declaration] is a wake-up call for the world that urgent action is needed,” said Dr. Nicole Lurie , executive director of emergency preparedness and response at the Coalition for Epidemic Preparedness Innovations, headquartered in Norway, in a statement.

Dr. Boghuma Titanji , assistant professor of medicine at Emory University, said in a statement that the declaration is "a crucial step toward enhancing coordination among African countries to address the ongoing mpox outbreak."

Why Africa is so worried

In the last couple of weeks, there has been a new and alarming development. Mpox has been detected in countries that have never previously identified cases. About 50 confirmed cases and more suspected cases have been reported in Burundi, Kenya, Rwanda and Uganda, according to World Health Organization officials.

It is with past health emergencies in mind that Africa CDC is trying to move quickly and garner international support.   "We were abandoned during COVID time, and today we don't want to be again abandoned. We don't want to be dependent. We are taking appropriate action,"   says Kaseya, noting that declaring a public health emergency is a new power that the African Union gave to the agency in 2023. Kaseya says that the agency sought input from more than 600 experts and that the scientific committee that was convened to consider the mpox situation unanimously recommended the emergency declaration.

Kaseya says it's particularly concerning that about 70% of cases in the DRC are in children under 18. "This one is a major alarm for the world," he says. "We are losing the youth in Africa."

Experts say the higher number of cases and deaths among children is likely because they don't have protection from the smallpox vaccine — which was discontinued after that related virus was eliminated in 1980 — and because about 40% of children in the region are malnourished, making it harder for their bodies to fight off the virus.

The U.S. response

There's concern about mpox in the U.S. as well. The U.S. Centers for Disease Control and Prevention issued an mpox alert last week. While the risk to the general population in the U.S. remains low, Christina Hutson, senior science adviser at the U.S. CDC, says it's important for clinicians, health departments and travelers to be aware of the virus's spread in Africa and be vigilant about symptoms.

In addition, last week the U.S. pledged nearly $424 million to help with what the U.S. Agency for International Development calls an " ongoing catastrophe " in the DRC, plus $10 million to respond to mpox and 50,000 mpox vaccine doses.

While Japan, the U.S., the European Union and vaccine manufacturers are working on vaccine donations, Africa CDC says the need far outstrips what's in the pipeline.

"We need to have vaccines. Today, we are just talking about almost 200,000 doses [becoming] available. We need at least 10 million doses," says Kaseya. "The vaccine is so expensive — we can put it around $100 per dose. There are not so many countries in Africa that can afford the cost of this vaccine."

The type of mpox spreading in the DRC's east — particularly among sex workers and other adults — and into some of the neighboring countries is a subtype called clade Ib. (Clade is the term used for mpox variants.) This is a new type of mpox that has kept scientists on their toes, discovering new information that is both good and bad.

It's harder for diagnostic tests to pick it up because of a genetic change in the virus, says Hutson of the U.S. CDC. It's also the first time clade Ib has been spread through sexual transmission. However, it also seems less fatal than the original clade I circulating elsewhere in the DRC. The number of people who have died has dropped below 1%. At least that, she says, is a glimmer of good news.

What is the Climate Like in Montenegro?

Montenegro has a Mediterranean climate near the coast and alpine mountains. In the summer, the average coastal temperature is in the high 20’s. Inland temps are cooler, usually around 10 degrees.

The Blue Cave is the largest of several natural excavations that can only be accessed by kayak. It gets its name from the changing colors of the water. It seems light blue in the morning, dark blue around noon, and again light blue before sunset.

After exploring the caves, you can either swim around in the blue waters or snorkel and explore the rocks and ocean life. If you are hungry or thirsty after a day in the sun there are plenty of local snack stands for you to choose from and enjoy. W.H.O. Declares Global Emergency Over New Mpox Outbreak

The rapid spread of mpox, formerly called monkeypox, in African countries constitutes a global health emergency, the World Health Organization declared on Wednesday.

This is the second time in three years that the W.H.O. has designated an mpox epidemic as a global emergency. It previously did so in July 2022. That outbreak went on to affect nearly 100,000 people , primarily gay and bisexual men, in 116 countries, and killed about 200 people.

The threat this time is deadlier. Since the beginning of this year, the Democratic Republic of Congo alone has reported 15,600 mpox cases and 537 deaths. Those most at risk include women and children under 15.

“The detection and rapid spread of a new clade of mpox in eastern D.R.C., its detection in neighboring countries that had not previously reported mpox, and the potential for further spread within Africa and beyond is very worrying,” said Dr. Tedros Adhanom Ghebreyesus, the W.H.O.’s director general.

The outbreak has spread through 13 countries in Africa, including a few that had never reported mpox cases before. On Tuesday, the Africa Centers for Disease Control and Prevention declared a “public health emergency of continental security,” the first time the organization has taken that step since the African Union granted it the power to do so last year.

“It’s in the interests of the countries, of the continent and of the world to get our arms around this and stop transmission as soon as we can,” said Dr. Nicole Lurie, the executive director for preparedness and response at the Coalition for Epidemic Preparedness Innovations, a nonprofit that finances vaccine development.

Facing the threat of global spread, the Centers for Disease Control and Prevention has urged clinicians and the public in the United States to be alert for the virus.

The W.H.O.’s designation of a “public health emergency of international concern” is intended to prompt member countries to begin preparing for the virus’s appearance and to share vaccines, treatments and other key resources with poorer nations.

“We need concerted international action to stem this recent, novel outbreak,” said Gregg Gonsalves, an epidemiologist at Yale University who served on the W.H.O.’s mpox committee in 2022.

The outbreak that year stayed mostly within tight sexual networks, among gay and bisexual men. A combination of behavioral changes and vaccination tamped down the spread.

In the United States, for example, the toll dropped to about 1,700 cases last year from more than 30,000 in 2022.

The version of mpox that has been circulating in Congo has always been more virulent, and currently has a death rate of about 3 percent, compared with 0.2 percent in the 2022 outbreak. The infection can produce fever, respiratory symptoms, muscle aches and swollen lymph nodes, as well as a rash on the hands, feet, chest, mouth or genitals.

Until recently, it spread mainly through consumption of contaminated meat or close contact with infected animals and people. Most of the deaths have been in children, who in this region of Africa are already beset by malnutrition and infectious diseases like cholera, measles and polio.

Should the outbreak spread globally, children in developed countries are likely to be less vulnerable to severe illness, experts have said.

Last year, for the first time, scientists discovered sexual transmission of this version of mpox, with cases split about equally among young men and women. According to genetic analyses , sometime around September, the virus gained mutations that enabled it to spread more readily among people. It has done so partly through heterosexual prostitution .

This viral type has not surfaced outside Africa. Over all, there have been more than 17,500 presumed and confirmed mpox cases in 13 countries, according to Africa C.D.C . Most of the cases and deaths have occurred in Congo.

Given the rapid spread, the declaration of a global health emergency was justified, said Anne Rimoin, an epidemiologist at the University of California, Los Angeles, who served on the 2022 mpox panel.

“I think we learned a great deal about the speed with which this virus can spread,” she said.

Amid rising numbers, Congo has approved two mpox vaccines , a Japanese product called LC16 and Jynneos, the vaccine made by Bavarian Nordic that was used in 2022 in the United States and Europe. But Congo has yet to institute an immunization plan.

On Aug. 9, the W.H.O. invited vaccine manufacturers to apply for an emergency use listing , a prerequisite for international groups such as Gavi, a global vaccine alliance, to purchase and distribute the shots in low-income nations.

Bavarian Nordic has donated 15,000 doses of Jynneos to be distributed in African countries. But that is a tiny fraction of the 10 million doses needed to control the outbreak, according to Africa C.D.C.

“The challenge is that these vaccines are not sufficient,” even if countries try to protect only the people at highest risk, said Dr. Dimie Ogoina, a Nigerian scientist and chair of the W.H.O.’s mpox emergency committee.

LC16 was used in Japanese children in the 1970s. Clinical trials funded by the Coalition for Epidemic Preparedness Innovations are now evaluating whether Jynneos can protect children and people who have already been exposed to the virus , Dr. Lurie said.

The organization is also supporting the development of a new mRNA vaccine made by BioNTech that would protect against mpox and related viruses, such as smallpox.

“This outbreak has been smoldering for quite a long time, and we continually have missed opportunities to shut it down,” Dr. Lurie said. “I’m really glad that everybody is now paying attention and focusing their efforts on this.”
